The wrapper diffs the entire ~/.codex/sessions tree before and after generation, then scans every new rollout file. A concurrent Codex session could be included and expose unrelated generated content or prompts.
The script collects all new rollout files by directory diff without a lock or unique session identifier. The README claims prior files are not touched, but the implementation can still include unrelated concurrent files.

External Codex CLI Invocation Uses Local Account
The script sends the generation instruction, raw prompt, and optional reference images to codex exec using the local logged-in account. This is expected for the skill, but it is a real external processing path for user data.
